COMPASS-CP
The CompassCP system is a SMART enabled application, that allows a care provider to assess the recovery process of a patient with a specific chronic condition and build a care plan for recovery. The care plan is based on a number of assessments performed by the care provider together with the patient. In these assessments the patient or care giver answers a number of questions describing the post discharge recovery. Each chronic condition has custom logic that allows to build a care plan based on the answers given by the patient. In the care plan a list of applicable local community resources are added based on the recovery path that a patient has to take and the proximity of the recovery organization to his/her recovery location.
